Go to  https://mission.consoloservices.com

Enter your USERNAME and PASSWORD

Ignore “YOU HAVE NO PATIENTS AT THIS TIME”

Using the SEARCH box in the top right hand corner, enter the last name of your patient and hit ENTER

Scroll down to the bottom and click on your patient’s name in BLUE (double check it’s the right patient)

Click on VOLUNTEER CARE NOTE SUMMARY

Click on CREATE A NEW VOLUNTEER CARE NOTE

Complete the following fields:

• EFFECTIVE DATE (the date of your visit / phone call)
• TIME OF EVENT (the start time of your visit)
• NOTE SUMMARY (a summary of your visit based on these CHARTING TIPS)
• VISIT TYPE (typically either HOME, PHONE or SNF Skilled Nursing Facility)
• VOLUNTEER (select your name from the list of volunteers)
• DATE IN
• DATE OUT
• TIME IN
• TIME OUT (round up, not down)

Click CREATE to save

When asked “DO YOU WANT TO ADD YOUR SIGNATURE?”  click YES

Go back through to make sure your note posted on the list.

Visit notes must be entered within 24 hours of your visit so the team can benefit from your findings.
